Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to join an in-house Skilled Nursing Facility in Waterbury, CT. The role supports quality resident care through comprehensive speech therapy services and rigorous documentation.

Position offers Full Time, Part Time, or PRN options with opportunities for growth within the organization. Must hold a Master's in Speech Language Pathology, CCC-SLP, and an active license in the practicing state(s).

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency is required.
• Active SLP license in state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patient, family, and staff on speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to identify residents' needs.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice

Continuing education/maintain licensure
