Evolve Therapy Services, LLC is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Maryland. The role supports resident care with direct speech therapy services and care coordination.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency (CCC) required or eligible.
• Active SLP license in the practicing state(s) required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete compliant CMS documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to identify resident needs and deliver clinical care.
