Job summary

Evolve Therapy Services is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility located in Prince Frederick, MD. This role requires providing high-quality speech therapy services, data-driven treatment planning, and patient-family education.

The ideal candidate holds a Master's degree in Speech-Language Pathology, active state licensure, and ASHA CCC certification, with ongoing commitment to professional development and CMS documentation in NetHealth EMR.

Qualifications

• Master's in Speech-Language Pathology required.
• CCC required for clinical eligibility.
• Must hold active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target lists and short/long-term goals.
• Educate patient, family, and staff to ensure carryover of therapies in daily activities.
• Complete CMS-compliant documentation in EMR systems (NetHealth).
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident needs screening with timely clinical care.
