Job summary

Creative Solutions in Healthcare - South in Houston, TX is seeking a full-time Speech Language Pathologist (SLP) to join our in-house rehab team delivering person-centered care and evidence-based therapy. The SLP will evaluate swallowing and cognition, implement individualized treatment plans, provide AAC systems, educate families and caregivers, and collaborate with other professionals on diagnostic procedures.

Active Texas license required; SNF/long-term care experience preferred.

Qualifications

• Active Texas license in Speech-Language Pathology
• In good standing with regulatory boards
• SNF/long-term care experience preferred
• Knowledge of current treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different personalities

Responsibilities

• Evaluate swallowing or cognitive deficits in residents
• Assist residents to improve cognition or swallowing goals
• Educate residents and caregivers
• Provide skilled speech-language therapy assessments and develop treatment plans
• Implement AAC systems for severe communication disorders
• Provide training to clients, families, and professionals
• Coordinate with other professionals for swallowing procedures
• Maintain competent clinical documentation and billing records
• Communicate changes in resident condition
• Coordinate with Restorative Nursing and staff
• Ensure confidentiality and compliance with laws
