Evolve Therapy Services is seeking a Speech Language Pathologist to support an in-house Skilled Nursing Facility in Oakdale, NY. The role is PRN and focuses on delivering quality speech therapy services, conducting evaluations, and developing targeted treatment plans to improve resident communication and swallowing functions.

The ideal candidate holds a Master's in Speech Language Pathology, CCC, active NY SLP licensure, and a commitment to ongoing professional development.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency required or eligible.
• Active SLP license in state(s) of practice required.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target list, and short- and long-term goals.
• Instruct patient, family, and staff to carry over therapy into daily activities.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to ensure resident needs are met timely and with clinical excellence.
