Job summary

Evolve Therapy Services in Hoosick Falls, NY seeks an experienced Speech Language Pathologist for an in-house Skilled Nursing Facility. This role offers Full Time, Part Time, or PRN options and focuses on delivering comprehensive therapy services to residents.

You will develop treatment plans, perform FEES/MBS oversight, document per CMS in NetHealth, and collaborate with staff to integrate communication goals into daily care. Active SLP licensure and CCC required.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Must have an active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and goals.
• Instruct patients, families, and staff to carry over therapies.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Participate in resident screening to meet needs timely and with clinical excellence.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice
