Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Hopkins, MN. This PRN position focuses on delivering high-quality speech therapy services and coordinating with CMS-compliant documentation using NetHealth EMR.

The ideal candidate will hold a Master’s in Speech Language Pathology, have CCC, and an active SLP license in the state. Ongoing CEUs are encouraged to maintain licensure and certification.

Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• Active SLP license in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate
• Evaluate and obtain data for treatment planning
• Develop treatment plans with target list and goals
• Instructs patient, family, and staff in speech-language procedures for carryover
• Complete compliant documentation in CMS expectations in NetHealth EMR systems
• Oversee FEES/MBS protocols and coordinate site programs
• Participate in screening protocols for resident needs with clinical excellence

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency
