Job summary

Evolve Therapy Services is seeking a licensed Physical Therapist to join our team for an in-house Skilled Nursing Facility in Le Center, MN. Roles include full-time, part-time, or PRN positions to deliver high-quality resident care.

You will evaluate residents, develop and implement treatment plans, and ensure accurate CMS-compliant documentation and billing within NetHealth EMR systems, collaborating with an interdisciplinary team.

Qualifications

• BS in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per established standards and productivity targets.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and expected outcomes.
• Communicate evaluation findings and treatment recommendations to residents, families, and the interdisciplinary team.
