Job summary

Evolve Therapy Services is seeking a licensed Physical Therapist for an in-house Skilled Nursing Facility in Muskego, Wisconsin. This position offers Full Time, Part Time, or PRN arrangements to support resident care and therapy services.

Responsibilities include evaluating residents, developing treatment plans, documenting in NetHealth EMR, collaborating with the interdisciplinary team, and ensuring CMS-compliant documentation.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Communicate findings and recommendations to residents, families and interdisciplinary team.
• Complete CMS-compliant documentation from CMS expectations in NetHealth EMR systems.
