Job summary

Responsibilities include providing PT services, documenting and billing per policy, conducting screenings, evaluating residents, and developing treatment plans with measurable goals.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound clinical judgment in evaluation, planning, and follow-up of therapy programs.
• Conduct regular resident screenings to determine intervention needs.
• Screen residents referred for PT to determine intervention requirements.
• Evaluate residents to collect data for treatment planning.
• Develop treatment plans with short and long-term goals.
• Communicate evaluation findings and recommendations to residents and families.
• Complete CMS-compliant documentation in NetHealth EMR.
