Job summary

Evolve Therapy Services in Burlington, WI is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility. This full-time position offers a sign-on bonus or PRN option, and involves planning, organizing, developing, and directing SLP services to meet CMS standards and resident needs.

Responsibilities include delivering quality therapy, evaluating data for treatment planning, creating goals, training staff and families, ensuring proper documentation in NetHealth EMR, and

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in the state(s) of practice.
• Continued education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan that includes a target list, short-term, and long-term goals.
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities.
• Complete CMS-compliant documentation from CMS expectations in NetHealth EMR systems.
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs.
• Participate in screening protocols for establishing needs of residents with clinical excellence.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice

Ongoing continuing education and professional development to maintain licensure and certification
