Evolve Therapy Services is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Paterson, NJ. This PRN role offers flexible Weekday or Weekend hours, with a focus on delivering high-quality speech therapy and meeting CMS documentation requirements.

The position requires a Master’s degree in Speech Language Pathology, CCC, and an active SLP license. Growth opportunities exist within the organization based on performance and need.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency required.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and collect data for treatment planning.
• Develop treatment plans with short- and long-term goals.
• Instruct patient, family, and staff for carry-over of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and site coordination.
• Participate in resident needs screening with clinical excellence.
