Evolve-Therapy-Services is seeking a Part Time or PRN Physical Therapy Assistant for an in-house Skilled Nursing Facility in Minot, North Dakota. The PTA will support the Physical Therapist, assist with screenings, evaluations, and treatment plans, and help coordinate patient schedules and care.

The role requires an Associate's degree in physical therapy, an active PTA license, and ongoing continuing education. Experience with CMS documentation is a plus and benefits are based on status.

Formación

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsabilidades

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying goals and treatment plans.
• Conduct individual and group treatment as outlined in the treatment plan.
• Train residents, families and staff in the use of therapy equipment.
• Document in NetHealth EMR as required by CMS.
