Evolve Therapy Services, LLC in Paris, TN is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility. This PRN position involves assisting the Physical Therapist, conducting screenings, and implementing treatment plans to maintain high quality resident care.

Responsibilities include documenting sessions in NetHealth EMR, collaborating with the therapy team, and supporting resident education.

Qualifications

• Associate degree completed from an accredited program.
• Active PTA license in state(s) of practice is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsabilités

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and help develop treatment goals and plans under the supervision of the Physical Therapist.
• Conduct individual and group treatment as outlined in the treatment plan.
• Assist in selecting techniques/media and adapting as indicated by resident performance.
• Monitor resident response and consult the Physical Therapist for modifications.
• Document compliant and medically necessary sessions in NetHealth EMR.

Formation

Associate's degree in physical therapy from an accredited program

Active Physical Therapy Assistant license in state(s) of practice
