Job summary

Evolve Therapy Services is seeking a licensed Physical Therapy Assistant to join an in-house Skilled Nursing Facility in New Haven, CT. You will provide therapy services under the supervision of a Physical Therapist, help develop treatment plans, and contribute to resident care.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in developing target lists and treatment plans with the Physical Therapist.
• Conduct individual and group treatments per the treatment plan.
• Assist in selecting techniques/media and adapt as indicated.
• Monitor residents’ response and modify as needed.
• Assist with resident scheduling and adherence to regimes.
• Educate residents and families and participate in QA programs.
