Job summary

Bradford Hills Nursing & Rehabilitation Center in Troy, PA, is seeking a dedicated Full-Time Physical Therapist (PT) to join our in-house rehabilitation team. This role focuses on improving mobility, strength, balance, and functional independence for residents in a skilled nursing and long-term care setting.

You will collaborate with physicians, nursing staff, and the rehabilitation team, develop individualized treatment plans, and ensure timely documentation while meeting regulatory standards.

Qualifications

• Current Pennsylvania Physical Therapist license or eligibility for licensure.
• Strong clinical, communication, and documentation skills.
• Commitment to patient-centered care.
• SNF/LTC experience preferred, but not required.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services to improve mobility, strength, balance, and functional independence.
