Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• CMS-compliant documentation in NetHealth EMR.

Responsibilities

• Provide PT services per established standards and productivity guidelines.
• Document and bill PT services per company procedures.
• Evaluate residents to develop treatment plans with short/long-term goals.
• Communicate evaluation findings and recommendations to residents, families, and the care team.
• Complete CMS-compliant documentation in NetHealth EMR.
