Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide PT services to residents per standards and procedures.
• Document and bill PT services according to procedures.
• Evaluate and develop resident therapy plans and goals.
• Screen residents to determine need for intervention.
• Communicate evaluation findings and plans to residents and team.
• Complete CMS documentation in NetHealth EMR as required.
