Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Muskego, WI. We offer Full Time, Part Time, or PRN positions to support resident care and program operations.

Responsibilities include evaluating residents, developing treatment plans, documenting per CMS and NetHealth EMR standards, and collaborating with the interdisciplinary team to deliver quality therapy services.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Maintain licensure and certification through ongoing development.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill therapy services according to CMS expectations.
• Evaluate residents to guide treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Communicate findings and recommendations to residents, families, and the care team.
• Contribute to interdisciplinary team discussions and care coordination.
• Complete compliant, medically necessary documentation in NetHealth EMR.
