Job summary

Qualifications

• BS in Physical Therapy required.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services in accordance with standards of practice and facility procedures.
• Adhere to documentation and billing procedures.
• Evaluate residents to develop and adjust treatment plans.
• Conduct timely screenings to determine need for intervention.
• Communicate findings and recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
