Job summary

Evolve Therapy Services is hiring compassionate physical therapists for an in-house Skilled Nursing Facility in Torrington, CT. The role supports planning, organizing, and directing PT services to ensure high-quality resident care in compliance with regulations.

Responsibilities include evaluating residents, creating treatment plans, documenting outcomes in NetHealth EMR, and communicating with families and the interdisciplinary team. Licensure in the state and ongoing CE are required.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents according to standards and procedures.
• Document and bill therapy services accurately per CMS requirements.
• Develop and implement treatment plans with short and long-term goals.
• Conduct screenings to determine intervention needs and adjust plans accordingly.
• Communicate findings and recommendations to residents, families, and the care team.
