Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Torrington, CT. You will plan, develop, and direct physical therapy services to ensure high-quality resident care.

You will provide PT services, document and bill correctly, evaluate residents, create treatment plans with short and long-term goals, and communicate results to residents, families, and the interdisciplinary team. Full-time, part-time, or PRN positions available.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Document and bill for physical therapy services.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Communicate evaluation findings and treatment recommendations to residents, families, and interdisciplinary team.
