Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Rochester, MN. This full-time (or PRN) role involves delivering resident-centered PT services with compliance to CMS and state requirements.

You will assess residents, develop treatment plans, document in NetHealth EMR, and collaborate with families and the interdisciplinary team to ensure high-quality, compliant care.

Qualifications

• Minimum BS in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development and high-quality resident care.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for PT services.
• Demonstrate sound evaluation, planning, implementation, and follow-up of therapy programs.
• Screen residents regularly to determine need for intervention/treatment.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, plans, and recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR as required.
