Job summary
Evolve Therapy Services is recruiting a Physical Therapist for an in-house Skilled Nursing Facility in Lowell, MA. This position offers Full Time, Part Time, or PRN options, with competitive pay and benefits.
You will provide PT services, assess residents, develop treatment plans, and collaborate with an interdisciplinary team. Required is a BS in Physical Therapy and an active state license; ongoing CE to maintain licensure; CMS documentation in NetHealth EMR; opportunities for growth within
Qualifications
- Bachelor of Science in Physical Therapy from an accredited program.
- Active Physical Therapy license in state(s) of practice.
- Ongoing continuing education to maintain licensure and certification.
- Commitment to CMS-compliant documentation and reporting.
Responsibilities
- Provide PT services per standards and productivity guidelines.
- Document and bill services per procedures.
- Evaluate and develop resident therapy plans.
- Screen residents to determine intervention needs.
- Communicate findings to residents and interdisciplinary team.
- Complete CMS-compliant documentation in NetHealth EMR.
- Perform other duties as required.
