Job summary

Avir at Terrell in Terrell, TX is seeking a Full-Time Physical Therapist to join our in-house rehab team. This role focuses on person-centered care to improve mobility, reduce pain, and restore function for residents in a supportive SNF setting.

Active Texas PT license required, with preferred SNF experience. In-house rehab, flexible schedules, and competitive benefits are available. New grads welcome to apply and collaborate with an interdisciplinary team.

Qualifications

• Active/Valid Texas PT license is required.
• Experience in SNF/long-term care is preferred.
• Familiarity with Net Health electronic documentation is a plus.
• Ability to work with diverse patient personalities.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering physical therapy treatments to relieve pain and restore function.
• Ensuring all treatments align with the plan of care.
• Monitoring patient response and adjusting treatment with IDT collaboration.
• Providing education to patients, families, and caregivers.
• Collaborating with interdisciplinary team to ensure quality care.
• Ensuring timely and thorough documentation per rules and policies.
• Attending required Rehab Director meetings.
