Job summary

City View Post Acute in San Francisco, CA, is seeking a compassionate Occupational Therapist (OT) to join our in-house rehabilitation team. Qualifications

• Must hold a California OT license or be eligible for licensure.
• New graduates are welcome to apply.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled interventions to improve ADLs, strength, balance, and independence.
• Address physical, cognitive, and functional limitations affecting daily activities.
• Train residents in adaptive techniques, energy conservation, safety strategies, and use of adaptive equipment.
• Educate residents and families on techniques to promote independence and prevent injury.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with nursing, physicians, and the interdisciplinary rehabilitation team.
• Complete accurate, timely, and compliant clinical documentation.
• Participate in care planning and discharge planning as needed.
