Job summary

Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Waverly, NE. The role supports comprehensive OT services within a partnering facility and reports to the administrator as part of the care team.

The OTA must hold active licensure in the state(s) of practice, possess an associate degree in OT, and participate in ongoing continuing education. Flexible full-time, part-time, or PRN shifts are available to meet facility needs.

Qualifications

• Must hold an OTA license active in the state(s) of practice.
• Associate's degree in Occupational Therapy.
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Screen residents to determine the best intervention or treatment.
• Implement treatment plans and monitor progress per plan of care.
• Communicate evaluation findings and recommendations to residents and care teams.
• Establish resident therapy schedules with the team.
• Ensure documentation and CMS billing for occupational therapy services.
