Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to provide in-house occupational therapy services at a Skilled Nursing Facility in Waverly, Nebraska. The role supports plan-of-care implementation and residents’ quality of life through direct therapy and collaboration with a multidisciplinary team.

Qualified candidates hold an active OTA license in the state of practice and engage in ongoing professional development to maintain licensure.

Qualifications

• Must hold active OTA license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment for each patient.
• Perform treatment plans and follow short- and long-term goals as per plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with team members.
• Conduct therapy according to the treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.
