Job summary

Evolve Therapy Services, LLC invites applications for a Certified Occupational Therapy Assistant at our in-house Skilled Nursing Facility in Bradford, PA. This is a part-time or PRN position requiring active OTA licensure.

You will plan, organize, and direct OT services, collaborate with the interdisciplinary team, and ensure proper documentation and billing in line with CMS standards. Experience in skilled nursing settings is preferred.

Qualifications

• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Screen regular residents to determine the best intervention or treatment.
• Perform treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy according to the treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily.
