Evolve Therapy Services is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Suring, WI. This full-time opportunity is complemented by part-time or PRN positions, with a sign-on bonus up to $7,500.

Responsibilities include conducting resident screenings, implementing treatment plans, documenting care, and collaborating with a multidisciplinary team to ensure quality resident care under CMS guidelines.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Perform treatment plan and follow short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families, and team members.
• Establish resident therapy schedules with team members.
• Document and bill occupational therapy services according to plan.
• Comply with CMS documentation standards daily.
