Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to join our in-house Skilled Nursing Facility in Fostoria, OH. The role requires applying OT services per plan of care under supervision, with a focus on resident quality of life.

You will screen, treat, document, and communicate with residents and families, schedule sessions with the care team, and maintain licensure and ongoing education.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Associates degree in Occupational Therapy required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment based on scope of practice.
• Perform treatment plans for residents; follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team.
• Establish individual resident therapy schedules with the team.
• Conduct therapy according to treatment plan and ensure proper documentation and billing of services.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.

Education

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice

Continuing education and professional development
