Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to provide in-house OT services at a Skilled Nursing Facility in Lyndhurst, Ohio. You will implement treatment plans, screen residents, and collaborate with an interdisciplinary team to optimize resident care.

Requirements include an Associate’s degree in OT and active OTA licensure. Ongoing continuing education is expected to maintain licensure and promote professional growth within a supportive facility.

• OTA licensure in applicable states.
• Associate's degree in Occupational Therapy completed.
• Commitment to ongoing professional development.

Responsabilités

• Screen residents to determine interventions and treatment.
• Perform and monitor treatment plans per care plan.
• Communicate evaluation findings and plans to residents and families.
• Establish and coordinate resident therapy schedules with the team.
• Document and bill OT services following CMS standards.
• Perform other duties as assigned to support patient care.
