Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in Fostoria, OH. The role supports full-time, part-time or PRN hours and follows established plans under supervision to ensure high-quality resident care.

The position requires an Associate’s degree in Occupational Therapy, active OTA licensure in the state(s) of practice, and ongoing continuing education to maintain licensure. CMS documentation is part of daily duties.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification required.

Responsibilities

• Screen residents to determine the best intervention for each patient based on scope of practice.
• Perform treatment plans following short- and long-term goals established by the supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with team members.
• Conduct therapy according to the treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice with daily point-of-care documentation.
