Job summary

Evolve Therapy Services, LLC is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Freeport, IL. The PRN position involves planning and directing OT services to ensure high-quality resident care in compliance with applicable standards.

Responsibilities include evaluating residents, developing treatment plans, coordinating schedules with the care team, and supervising staff as needed.

Qualifications

• Active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans and goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education

Degree in Occupational Therapy

Active Occupational Therapy license

Continuing education to maintain licensure
