Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Freeport, IL. Full-time, part-time, or PRN positions are available to support resident care across shifts.

The OTA will plan, organize, develop, and direct occupational therapy services in accordance with federal, state, and local standards, ensuring high-quality resident care at all times.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Perform treatment plans following established goals and plan of care.
• Interpret and communicate evaluation findings, plans, and recommendations to residents and families.
• Establish resident therapy schedules with team members.
• Conduct therapy according to treatment plan and document services for billing.
• Perform all duties to effectively treat patients as assigned.
• Comply with CMS documentation standards daily.
