Job summary

Evolve Therapy Services is seeking a dedicated Occupational Therapist to provide in-house therapy services at our partner facility in Trotwood, OH. This PRN position requires planning, organizing, developing, and directing OT services in accordance with current standards, and collaborating with an interdisciplinary team to ensure high-quality resident care.

Qualifications

• OT degree from accredited program.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screenings of residents to determine best intervention.
• Evaluate residents to inform treatment plans and set short- and long-term goals.
• Interpret and communicate evaluation findings and plans to residents and families.
• Establish individual therapy schedules with team members.
• Deliver therapy according to treatment plan and document/bill services.
• Perform other duties as assigned to treat patients.
• Oversee COTA as needed and ensure documentation and reimbursement practices are followed.

Degree in Occupational Therapy

Active Occupational Therapy license
