Job summary

Evolve Therapy Services, LLC is seeking an Occupational Therapist for an in-house Skilled Nursing Facility located in Key West, FL. The role supports ongoing therapy services with a focus on quality resident care and compliance with applicable standards.

Responsibilities include evaluating residents, planning treatment, coordinating with families and interdisciplinary team, and supervising COTA when needed. Licensing is required and ongoing CE credits are expected.

Qualifications

• OT degree required
• Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Conduct regular resident screening and determine best intervention for each patient
• Evaluate residents and develop short- and long-term treatment goals
• Communicate evaluation findings and treatment plans to residents, families, and the inter-disciplinary team
• Establish individual therapy schedules with team members
• Deliver therapy per plan and ensure proper documentation and billing
• Perform other duties to effectively treat patients
• Oversee COTA and ensure documentation and treatments meet clinical and reimbursement standards
