Job summary

Mission Park Healthcare Center in Santa Barbara, CA is actively seeking a Full Time & PRN Occupational Therapist to join our in-house rehab team.

You will evaluate patients, develop treatment plans, document care, and collaborate with therapists and nursing staff in a facility known for compassionate, ethical care.

Qualifications

• Evaluate patients and determine the best course of therapy.
• Develop individualized treatment plans based on physician referrals.
• Document all clinical and regulatory information accurately.

Responsibilities

• Evaluate patients in a timely manner following physician referral.
• Develop effective treatment plans and provide education for client, family and caregivers as subject matter expert.
• Communicate with supervisor and other health team members regarding patient progress, problems and plans.
• Participate in inservice training program for other staff in the facility.
• Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy.
• Provide accurate and timely billing records and supporting clinical documentation.
• Instruct patient's families or nursing staff in maintenance programs.
• Coordinate necessary durable medical equipment and adaptive equipment for patients to facilitate safety and independence in ADL's.
• Assure resident abuse or neglect does not occur.
• Observe confidentiality and safeguards for all patient information.
• Complies with federal, state, and local legal and professional requirements.
