Flagship Therapy's Solimar Post Acute in Oxnard, CA is seeking a dedicated Occupational Therapist to join our in-house rehab team. This full-time role offers a flexible schedule, a supportive work environment, and compensation of $55–$65 per hour based on experience.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop a care plan.
• Use standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled and medically necessary treatment per requirements.
• Administer ADL retraining, therapeutic activities, exercises and modalities as part of rehab.
• Assess progress and adjust treatments in the clinical record per requirements.
• Evaluate treatment outcomes and refer as needed based on outcomes.
• Provide consultation to patients, families, caregivers and other providers.
• Communicate progress, barriers and plans with supervisor and health team.
• Provide discharge summaries per clinical requirements.
