Job summary

Mercy Home Health Care, an agency of Trinity Health At Home, seeks a Full-time RN WOCN Consultant to be based at our Newtown Square office serving Delaware and Chester Counties. You will provide one-to-one wound care and coordinate treatment plans to achieve defined goals, implementing therapies and evaluating outcomes in home settings.

The ideal candidate holds PA RN licensure and is WOCN-certified or a prepared WOCN.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the state of PA.
• Three years of professional care nursing experience.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Provide one-to-one wound care to Mercy Home Health patients.
• Coordinate treatment plans, goals, implementation, and outcomes.
• Educate patients, families and staff on wound management and prevention.
