Job summary

Mercy Home Health Care in Philadelphia is seeking a Full-time RN WOCN Consultant to join our home-based care team. This role emphasizes one-to-one patient care, wound management, and coordinated treatment plans in patients’ homes.

You will provide education to patients, families, and staff; ensure evidence-based wound care, pressure ulcer prevention, and appropriate dressings. Active PA RN license and WOCN certification (or prepared) are required, with reliable transportation and a valid

Mercy Home Health Care has a need for a Full-time RN WOCN Consultant.

This position is based from our South Philadelphia branch office and will support the City of Philadelphia.

Responsibilities

• The WOCN is responsible to provide wound care to the patients of Mercy Home Health.
• Coordinate treatment to provide comprehensive wound care assessments and plan of care including goals, implementation of treatment, evaluation and outcomes for patients.
• Provides patient, family, and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections, and other therapies.
• Current RN licensure with WOCN required or Certified prepared wound, ostomy, and continence nurse (WOCN).
• Valid driver’s license and car insurance.
• Strong interpersonal and communication skills.
• Demonstrates ability to use problem solving, critical thinking and priority setting skills.

Qualifications

• Graduate of an approved nursing education program
• Licensure as a Registered Nurse in the state of PA
• One (3) years experience as a professional care nurse
• Must have current Driver’s license and reliable transportation
