Job summary

Advantmed is seeking licensed Nurse Practitioners or Physician Assistants to conduct structured in-home wellness and risk adjustment assessments. This PRN role offers consistent visit availability, training, and supportive coordination for a reliable income stream.

Providers can expect 30+ hours per month with a ~55-mile travel radius, paid mileage, and a strong operational framework. This is a 1099 PRN engagement focused on quality care and documentation.

Qualifications

• Active NP or PA license in applicable state
• 3+ years patient care experience preferred
• Comfortable with EMR and independent field-based work
• In-home or risk adjustment experience preferred

Responsibilities

• Conduct in-home Annual Wellness Visits and chronic condition assessments
• Perform focused histories, physical exams, vitals, medication reviews and screenings
• Document visits in the EMR to support value-based care and risk adjustment
• Operate within NP/PA scope of practice and state regulations
• Travel to assigned counties within a ~55-mile radius
