Job summary

VitalCaring Group is seeking a Speech-Language Pathologist to deliver in-home evaluations and therapy, focusing on speech, language, cognition, and swallowing. This field-based role emphasizes autonomy with clinical leadership support and patient-centered outcomes in a home-health setting.

You will conduct assessments, develop plans, and use evidence-based approaches to improve communication and safety for patients in their homes. A current state license and CCC‑SLP preferred.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology.
• Current SLP license in the state of practice.
• CCC‑SLP is preferred.
• CPR certification is required.

Responsibilities

• Conduct comprehensive evaluations of speech, language, cognitive, and swallowing function.
• Develop and implement individualized plans of care to improve communication, cognition, and safety.
• Provide therapy using evidence‑based techniques and specialized tools.
• Assess home environment and recommend adaptive equipment or communication aids.
• Educate patients and caregivers on exercises, compensatory strategies, and safety techniques.
• Monitor progress, reassess, and adjust treatment plans as needed.
• Document all care accurately within the EMR.
• Collaborate with nurses, therapists, and interdisciplinary teams to ensure coordinated care.
• Participate in case conferences, discharge planning, and ongoing care coordination.
