Job summary

MHH Clinical Services PLLC is seeking licensed Speech-Language Pathologists to provide one-on-one speech therapy in patients' homes across NYC. Cases are available throughout Brooklyn, Queens, Manhattan, the Bronx, and Staten Island with assignments based on location and availability.

Responsibilities include comprehensive evaluations and developing goal-directed plans, educating families, collaborating with the care team, and maintaining proper documentation.

Qualifications

• Current New York State Speech-Language Pathologist license.
• Experience working with pediatric and/or adult populations.
• Home care and/or community-based experience is a plus.
• Experience with ID/DD is preferred.
• Strong clinical assessment and communication skills.

Responsibilities

• Provide one-on-one speech therapy services to children and adults in patients' homes.
• Conduct comprehensive evaluations of speech, language, cognition, voice, fluency, and swallowing.
• Develop, implement and update individualized treatment plans per physician orders.
• Educate patients and families on strategies, home programs, and carryover techniques.
• Collaborate with families, physicians, and inter-disciplinary team for continuity of care.
• Monitor progress and modify plans as needed.
• Maintain compliant clinical documentation.
• Participate in meetings and care coordination when required.
• Adhere to professional and organizational standards.
