Job summary

Excellent Home Care Inc. is seeking a dedicated Speech Language Pathologist (SLP) to join our home health team in the New York City area.

You will plan and deliver speech and language rehabilitation in patients’ homes, coordinate with family and health professionals, and maintain comprehensive patient records. The ideal candidate has a Master’s in Speech Language Pathology, at least 1 year of clinical SLP experience in health care, and NY licensure or eligibility.

Qualifications

• A minimum of one year Speech Language Pathology experience in a Health Care setting.
• Demonstration of considerable knowledge of current standards, procedures and techniques of Speech Language Pathology. Home Care experience desirable.
• Currently licensed in New York State as a Speech Language Pathologist or eligible for license.

Responsibilities

• Planning and provision of Speech and Language Rehabilitation services to Patients in their homes.
• Work with other professional staff to establish rehabilitation and maintenance goals and activities.
• Evaluates needs and provides rehabilitative services as ordered by the Patient’s Authorized Medical Practitioner.
• Performs clinical tests to assess Speech and Language disorders and develops a Plan of Care with goals.
• Instructs Patients/Family in the use of assistive modalities and procedures.
• Maintains records and discharges summaries for patient records.
• Communicates with the Nurse coordinating care and reports progress.
