Job summary

CenterWell Home Health is seeking a licensed Speech-Language Pathologist to deliver in-home or facility-based evaluations and therapy. You will develop and review care plans, administer assessments, and provide training to patients and families while documenting progress and coordinating care with the team.

The role requires at least six months of SLP experience, an unrestricted license, CPR certification, and a valid driver's license with adequate auto insurance.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
• Observe, record and report changes in the patient's condition and response to treatment to supervisor/physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company; prepare progress reports and care plans.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and follow company policy on information sharing.
