Job summary

VitalCaring Group is seeking a Home Health Speech-Language Pathologist to evaluate and treat patients in their homes, restoring communication, cognition, and swallowing function. You will lead one-on-one care in a field-based role, maintaining autonomy while collaborating with a supportive clinical leadership team.

The role requires a Master’s or Doctoral degree in Speech-Language Pathology, a current SLP license, and 1+ year of clinical experience (home health preferred).

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology.
• Current SLP license in state of practice.
• Minimum 1 year clinical experience (home health preferred).
• CCC-SLP certification preferred.
• Strong clinical judgment and ability to work independently.
• Excellent communication and patient education skills.

Responsibilities

• Lead evaluation and treatment of patients with speech, language, cognitive, and swallowing disorders.
• Develop and implement individualized plans of care to improve communication, cognition, and safety.
• Provide therapies using evidence-based techniques and specialized tools.
• Assess home environment and recommend adaptive equipment or communication aids.
• Educate patients and caregivers on exercises and safety techniques.
• Monitor progress, reassess, and adjust treatment plans accordingly.
• Coordinate with physicians and care team for changes in condition.
