Job summary

VitalCaring seeks a Home Health Speech-Language Pathologist to evaluate and treat patients in their homes, focusing on speech, language, cognition, and swallowing. The role emphasizes autonomy and collaboration with a dedicated care team.

You will deliver individualized plans, monitor progress, and adjust treatment as needed, while traveling between patient homes to provide compassionate, evidence‑based care.

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology.
• Current SLP license in the state of practice.
• CCC‑SLP preferred.
• At least 1 year clinical experience; home health preferred.
• Strong clinical judgment and independent work ability.
• Excellent communication and patient education skills.
• Comfortable traveling between patient homes.
• Current CPR certification.

Responsibilities

• Conduct comprehensive evaluations of speech, language, cognitive, and swallowing function.
• Develop and implement individualized plans of care to improve communication, cognition, and safety.
• Provide therapeutic interventions using evidence‑based techniques.
• Assess the home environment and recommend adaptive equipment or communication aids.
• Educate patients and caregivers on exercises, compensatory strategies, and safety.
