• Strong communication and interpersonal skills to engage with patients, families and caregivers.
• Ability to perform comprehensive assessments of speech, language, cognitive-communication, and swallowing disorders.
• Proficiency in documenting observations, diagnoses and treatment plans clearly and accurately.

Responsibilities

• Conduct home visits within designated areas.
• Perform assessments to diagnose speech, language, communication, and swallowing issues.
• Develop, implement, and revise personalized treatment plans based on evaluations, physician’s orders, and client goals.
• Provide rehabilitative care and patient, family, or caregiver education to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations using electronic medical records.
