Job summary

CXQX Fox Rehabilitation Services WI LLC is seeking a passionate Speech Language Pathologist to deliver in-home and community-based care in Menomonee Falls, WI and surrounding areas. Therapies focus on geriatric patients with autonomy and flexible scheduling through the Geriatric House Calls model.

You’ll conduct evaluations, create care plans, and provide evidence-based treatment, including telehealth when appropriate, while collaborating with an interdisciplinary team to improve patients’

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice.
• Degree from an accredited Speech Language Pathology program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in home or community settings.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to support wellness, longevity and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
